The Unisa application form is a document that the University uses to collect data from applicants who are applying to the University for the first time for 2023-2024. The application can be in two different versions. The form can be online based (soft Copy) or it can be obtained from the central office (main Office) for the hard copy. On the application form, applicants need to provide some personal information like their full names, age, sex, result slips (grade) and so on.

ALL applicants who wish to apply for admission to study at the University of South Africa (UNISA) should visit the www.unisa.ac.za forms page and download a copy in pdf format. However, if you want to print the form into a hard copy, you can do that by following the process below:

  • Visit the UNISA website at www.unisa.ac.za
  • Click on Apply for Admission at the top menu of the page
  • Then, choose whether you are a first-time applicant or returning student
  • After that, you select the qualification level
  • Download and complete the forms online
  • Then, submit your application form online for review

Documents Needed to Fill out The Unisa Application Forms For 2023

To be able to complete the application forms of Unisa for 2023, you will need the following documents, as the information on them is needful:

  • A copy of your School Certificate (Senior or National Senior Certificate)
  • A copy of your ID documents (RSA Applicants) and Passport ID (International Applicants)
  • A copy of your Official Tertiary Academic Records if required
  • A Sworn Translation of documents if not in English or Afrikaans
  • A copy of your Marriage Certificate or Divorce decree if applicable

Login to myUnisa. Click on “myAdmin”, “Edit Registration”, “Request Registration Letter” and “Request Letter”. A secure PDF document will be sent to your myLife e-mail account. Use your ID number/passport number to open the secure PDF.
DSAR02 To request the addition/cancellation of study units, log in to myUnisa. Click on “myAdmin” and “Edit Registration”.
DSAR03 To change your address and contact numbers, login to myUnisa. Click on “myAdmin”, “Biographical Details” and “Update Address Details”.

You can request your academic record via myUnisa. Login to myUnisa. Click on “Academic Record” and “Code”. Your academic record will be sent to your myLife e-mail account.

Master’s and doctoral students

  • You can request your academic record via myUnisa. Login to myUnisa. Click on “Academic Record” and “Code”. Your academic record will be sent to your myLife e-mail account.
  • You may also request your academic record by completing the Application for academic record or credit certificate (CGS08) (PDF 100KB) for masters and doctoral students. Please submit your completed CGS08 form to mandd@unisa.ac.za
